Sports Registration Packet

To register your child for the 2003-2004 sports season, please complete the following steps:

• Download the Sports Registration Packet (450 Kb pdf)
• Complete the sports Registration form (one per family).
• Complete the Medical Information form (one for each child.
• Sign the Archdiocese of Chicago release form
• Enclose the sports fees
• Return completed forms and fees by May 30, 2003
